Phentermine Hydrochloride, known as Adipex, is a weight loss medication and appetite suppressant allowed for short-term use in conjunction with a program of exercise, caloric restrictions and behavioral modification. Generally the drug is prescribed for only 3 months at a time, though some patients take it for 3 months, stop for a few months and begin taking it again.
Adipex was also one of the ingredients in the now-infamous Fen-Phen diet pill which was pulled from the market and prohibited by the FDA because of it's dangerous effect of causing heart valve abnormalities.
So, it seems not too surprising that Adipex is used with caution. There are several reasons why the drug has restricted use.

As stated, Adipex can have adverse effects on your cardiovascular system, since it is known to be connected to heart valve disease and high blood pressure. The drug speeds up the heart rate and has an effect on the central nervous system and, for this reason, is used with caution with those with a history of anxiety too, and shouldn't be used if you have trouble sleeping.
If you have a prior history of cardiovascular disease, high blood pressure, stroke or congestive heart failure, you should not use Adipex. Those with borderline high blood pressure can develop high blood pressure if they use the drug.
The other problem with Adipex, being similar to other stimulants like Amphetamine and other appetite depressants, is that it can be addictive, especially when there is long-term use. Withdrawals include excessive drowsiness, fatigue, shaking and depression.
Related to this dependence on the drug is the tendency to develop a tolerance for it. It simply may not work after awhile. And once you've developed this tolerance for it, you are subject to its harmful effects without getting its benefits. This is another reason why the drug is only prescribed for a limited amount of time.

While it is important to be in shape, and medications might be necessary in extreme cases when health is at risk, it seems even with the use of prescriptions, other measures should be taken for healthy weight loss: Including exercise, healthy eating and possibly counseling. No pill is the end-all or ultimate solution for problems of obesity. It is best to get to the root causes of the problem than to only seek the supposedly easier or quicker solution that is short-term at best.
