12 Years Boys Have No Acne

Children are free from acne until puberty, when all the body activities
are directed towards building more bones and muscles, and preparing the body to reproduce (to preserve life).The pituitary gland (maestro gland) hormones direct the body to serve these purposes, thus :

• Growth hormone increases utilization of fats for energy production, increases lipolysis (fat breakdown by fat cells), spares carbohydrate, and increases protein synthesis.

• Thyroid Stimulating Hormone(TSH) stimulates the thyroid gland to secrete Thyroid hormones, and these increase oxygen utilization and metabolic rate, as well as they increase glucose absorption.

• Follicle Stimulating Hormone (FSH) and Lutenizing Hormone (LH) causes
increased bio-synthesis of sex steroids by the ovaries and testis. Sex hormones build muscles and raises blood glucose so as to be maintained within a certain range.

The net result is increased energy production from fats, decreased energy production from carbohydrates (as it is needed for protein metabolism) , increased protein synthesis, and increased bio-synthesis of sex hormones. The biochemical translation of that is, the production of huge amounts of COENZYME A.

Coenzyme A and Acne

Coenzyme A is the central molecule in our metabolism, it combines acetate that comes from carbohydrates, protein, and fat metabolism to form Acetyl-Coenzyme A, the only molecule that the mitochondria use to produce energy through Krebs cycle. Also fat synthesis - including steroidal hormones - starts with Acetyl Coenzyme A. Acetyl choline the neurotransmitter of all para-sympathetic nervous system can not be synthesized without acetyl-coenzyme A.

Coenzyme A is formed by stepwise union of Cysteine (amino acid), ATP
(high energy molecule), and Pantothenate (Vitamin B5). ATP is provided by metabolism, Cysteine if not present in food can be derived from Methionine (amino acid), and
Vitamin B5 can be only provided by food.

The body deals with the high demand of an essential nutrient in such a way to maintain the functionally more important reactions, while at the same time slowing down the less important ones (or why a certain disease is debilitating?).

In our case, where Coenzyme A is a pivotal molecule in at least 70 metabolic pathways, the body chooses to utilize Coenzyme A and excess fats to build sex hormones, leaving excess fats unprocessed and stored. Were there extra amounts of Coenzyme A, the body would metabolize the excess fat in the sebaceous glands.

Now we can easily understand that acne is due to un-metabolized fat that is accumulated in the sebaceous glands, due to insufficient amounts of Coenzyme A,i.e., acne is due to slow metabolism of excess fat.

Why It Is Hormones Biosynthesis and Not Androgen Effect

The reason acne first erupts at puberty is not effects of hormones, but rather metabolic, it is secondary to deployement of a substantial amount of coenzyme A to the purpose of synthesis of sex hormones. In the following examples we may see that, it is not the androgen that is the cause of acne:

• Eunuchs (early castrated males) rarely exhibit acne, while normal adult males may exhibit adult acne

• Two groups of adolescent boys with the normal blood levels of androgens may exihibit different incidence of acne

• Stress related acne ( prior to examination, tension, insomnia...etc)
is caused by increased synthesis of corticosteroids that the body secretes to combat stress. The same thing apply for adult acne of people who have not had acne as teenagers

• The premenstrual flare of acne patients due to increased levels of progesterone which is not an androgen

• Rapid decline in incidence of acne after puberty is due to cessation of need of increased sex hormones synthesis

• Biotin and Nicotinamide are important vitamins of lipid metabolism, and with vitamin B5 they may help cure acne, yet when they are used alone, they are far less effective in treating acne

• Frequently seen acne with those secreting tumors of the ovary, testis and adrenals, in people with variant ages

Excess Fat


Acne Cure With Coenzyme A

The precursors of coenzyme A are ATP which is very abundant in the body of adolescent people, Cysteine which is non essential amino acid that can be supplied with food or derived from the essential amino acid Methionine, and Vitamin B5 (Pantothenic acid) which is essentially supplied by food.

Pantothenic acid (vitamin B5) supply to increase body production of coenzyme A , has been recommended by Dr Lit-Hung Leung in high doses reaching 1000-1500 times its recommended daily intake which is 10 milligrams per day. The recommended dose is so high (10-15 grams/day), because vitamin B5 is a water soluble vitamin, i.e., with each heart beat the unused B5 molecules pass into the kidney to be excreted. It follows that Vitamin B5 should be present all the time to capture any free Cysteine.

Add Cysteine to Reduce Pantothenic Dose

Since the demand for coenzyme A increases in adolescence, the need for Cysteine must increase. Cysteine is a sulfur amino acid that is heavily used in building keratin, which is an important constituent of the skin (which increases largely in size during adolescence), growing wanted and unwanted hair, and also used in forming nails.

Cysteine free pool in the liver has a narrow range, since the liver cells tightly regulates it to keep it within that range. The liver synthesises Glutathione as a reservoir of cysteine. Glutathione is the primary tool of the liver for detixification. It has a vital role in many cell cycle related events that are needed during tissue repair. It is also vital for maintaining functioning of the immune system.

Cysteine with Glutamic acid (another amino acid) are considered by some researchers to be essential elements for adolescents.

Administration of Cysteine is the only means by which we can reduce the dose of Pantothenic acid. I have recommended the combination of Pantothenic acid + Cysteine for many acne patients for more than 10 years with great results in curing acne.

Taking 0.5-1 grams of Cysteine per day reduces your intake of Pantothenic acid to only 1-1.5 grams per day (they will be combined).

Pantethine, The New Relief

The metabolic pathway of synthesis of coenzyme A involves a compound called Pantetheine which is the direct precursor of coenzyme A. It is now produced in the stable form pantethine, as it involves 2 molecules in the disulfide form of pantethine. It has an extra advantage of being sustained over time, since every molecule has to be split first, and this delays its elimination.

It was intensively researched for its cholesterol and lipid lowering properties (aren't they excess fats?), and has become very popular for cardiovascular support. The literature about Pantethine is full of details about that property, and this made me reluctant to recommend its use for acne, until I found some people are already using Pantethine to cure acne as you can read in the link below.

The dose of Pantethine in most human studies is 900 mg per day and this may be a good guide for us to start with 300 mg three times daily until improvement, it is supposed to improve acne within 2 days.

1 month is a sufficient time to improve the case and being safe at the same time.

Usually the supplements are taken in the range if 1-2 g/day but since b5 was taken in high doses reaching 10g/day, it is difficult to discriminate the optimal dose.

As for L-Carnitine I don't recommend it since it caused aberrations of treatment in some cases.

I looked into it and from the threads, it's really difficult to tell because the guys use was so inconsistent with their doses. But they were using a 2:1 ratio of B5 to L-Cysteine. I guess the important question is: What is the highest "SAFE" does of B5 when taking it with L-cysteine?

Also, do you think that the addition of L-carnitine might make the B5 even more effective, thus allowing a lower effective B5 dose?

But please let me get this strait. You are saying that B5 in high doses lowers L-Cysteine, and low L-Cysteine is responsible for hair loss and oily skin or just hair loss? I am also trying to figure out the connection between liver function and oily skin. My best guess is that a poorly functioning liver is not filtering out all of the used/broken down sex hormones and it's the extra sex hormones floating around that are responsible for the oily skin (among other things). Am I on track here?

Also, there are some who are claiming that B5 also depletes copper (in addition to L-Cysteine) and that copper depletion could be partially responsible for B5 relate hair loss. Of course if you supplement copper, you also have to supplement Zinc because they basically compete. I would love to hear your thoughts on all of this because I honestly don't know what to make of it.

For maintenance you may keep using these supplements whenever acne shows up, I can not recommend that you take anything for life.

As for the liver detox, it is hoped that it may help the liver to maintain L-Cysteine pool in the healthy range. L-Cysteine is an example of a tightly controlled amino acid that is widely used in structural biochemical reactions, like formation of hair, nails, hormones, enzymes ...etc., this pool is regulated by the liver.

Update: It's now day 10 and I am starting to get less oil, so it seems to be starting to work. My (hopefully) last questions are: 1) How long is it safe to stay at this B5 dose (6 grams B5 with 3 grams L-Cysteine daily)? and 2) What dose should I drop down to to maintain (for the rest of my life I suspect)?? 3) What is the highest SAFE maintenance dose???

I am already taking L-Cysteine with B5 (and biotin) for 8 days now with only minimal results. I started at 10 grams of B5, but soon lowered the does after finding many people online complaining of hair loss symptoms even after stopping the B5 for many years. Since I am not seeing any real significant improvement in my oily skin / relentless cystic acne problem, what specific dose of L-Cysteine to B5 do you recommend????

I am currently taking 2 grams of B5 (3x/day) with 1 gram of L-Cysteine (also 3x/day) for a total daily dose of 6 grams of B5 and 3 grams of L-Cysteine, but I would love to hear your suggestion. Thanks again, you have been very informative.

1- You can avoid hair loss by reducing the dose of vitamin B5

2- The optimal dose of vitamin B5 is 1-2 grams/day

3- The mechanism of hair loss is unknown but it may be induced by reduction of the amounts of l-Cysteine in the blood (to form Coenzyme A), l-Cysteine is needed for building keratin which the main constituent of hair.

Just a quick update: this regime has definitely cleared my skin. Very pleased. To those who are undecided about trying it, I would say 100% give it a go. It works. (7 grams of PA and 1.5 grams of L-Cysteine).

I've got 4 weeks before I start to reduce the amount of PA I take. Bit nervous about this in case the acne returns. But, for the moment, it feels incredible to have skin that is 100% clear (even with a diet that includes caffeine and dairy).

Hello everyone

A quick update: I have had a very good 2/3 weeks since I increased my dosage levels to 7 grams of PA with 1.5 grams of L-Cysteine. I get the occasional spot but it's 100% better. I plan to keep it at these (high?) levels for 2 months and then try to reduce, slowly, the amount of PA I take. My acne is cyclical so a cynic might say that this currently excellent period is just my body in a good part of its cycle. I do however think that this regime work is working for me. What do you recommend between B5 and Pantethine for someone who never try any of these?

I was on Accutane for 4 courses, all on low does. Been 1.5 years now and acne is bounching back. I am using topical Paula's Choice CLEAR program and it's working. However, new pimple occurs every day, about 1-2, although small I am unhappy with red marks it left behind.

Because of new pimple every day, I want to reduce oil production. Actually I can go back on Accutane but its toxicity is much unwanted.

FYI - I also lose about 40-60 hair / day.

- 6 grams of Pantothenic Acid; and

Been about 2 weeks on this regime. Getting (still) 1 new spot each day. My spots take a very long time to fade (despite trying to maintain a good diet and drink lots of water). I am very impatient as I've been fighting acne for about 20 years. I am anxious that my acne isn't responding as I'd hoped.

My questions are: do you think my balance between PA and Cysteine is about right (6 g :1.5 g)? Would you recommend my using Penaten on existing spots and/or adding Pantetheine tablets? (Is Penaten just to be used on existing spots or should it be applied each morning and evening as a matter of course to prevent lesions appearing?)

Hi.I was contemplating my 4th course of Roaccutane before reading this hub.

I'm a 33 year old man. About to start on this. Just a little confused by the discussion sbove (as a non-scientist). Planning each day, initially, to take:

-3 x 1gram of PA; with

-3 x 250mg of cysteine.

Do I need to take a b-complex and/or panthenine? Will the above dosage levels be too low to bring about results? Is the above a good starting point?

Glad I didn't give up as I can say I'm 90% clear now.

I up my doses to 2 gram L-cysteine, 6 gram B-5, and 900 mg Jarrow Pantethine a day. For the first 2 month, my crystal acne disappear but kept appearing in another spot, I was going to give up but was going to keep taking all the pill until I ran out. The 3rd month, I started getting vulgaris acne since my skin became more dry, but no more crystal acne, my skin still has lots of old red acne mark that looks like there are still acne there. Month 4-5 my acne starts fading away and the red mark on my forehead and cheek started fading but by neck and around my jaw was still red. I was running low on the pill, so I decided to stock up for another 2 month. Month 6-7, the red mark are clearing up and when I get a pimple, it usually disappear completely in 1-2 days.

SOOOOOOOOOOOOO Happy! Thank you sir, you save my life, now I can concentrate on finishing my degree and move on to a brighter future. Also, my lip are getting dry, don't know if its the heater or not, but I'm going to start lowing my dosage now, think I will lower b-5 to 4 gram first.

Ive taken pantothenic acid many times on and of n all i have to say is it doesn't take more than 4 days for your acne to completely disapear,,, TRUST ME, panthenol has mixed reviews meaning it doesn't work nor have any connection with pantothenic acid, they just want you to buy their crap,, stick with panto. Acid i been using it for a while, proactive clearasil All crap compared to this

I started out taking

Two - Vitacost L-Cysteine -- 500 mg

I'm using Bepanthen and I saw this cream has other ingredients than Penaten (beside 5% dexpanthenol). But it is also a geasy cream. Until now I used it only for my back in combination with Brevoxyl (4% bpo). I'm applying Brevoxyl to kill p.acnes and after 20-30 minutes I'm applying Bepanthen. After 5 days I can say I really see a difference. I'm not seeing new zits forming but i have many marks from previous ones (i'm having bacne for years now). It;s a good result for only 5 days of treatment and I will continue at least 1-2 months with it.

The cream is way too thick to use i tryed it one night and does not properly absorb into the skin, it is not made for the face, I could use it at night but not a chance in the morning way to thick.. I believe myself creams are not the answer, I dont see how a cream can stop your skin from producing oil, I started 6 tabs(450mg) yesterday i'll give it a few weeks but im optimistic, dryer lips is all i can feel at the moment. Are u using the cream any results?

It contains: aqua, panthenol 5%, isopropyl myristate, cetyl alcohol, stearyl alcohol, propylene glycol, lanolin, potassium cetyl phosphate, pantolacton, phenoxyethanol, potassium hydroxide. I read the intructions and it says that this cream is greasy, suitable for dry skin. I'm in a dilemma because I have oily skin and this combination (my face + bepanthen) can have comedogenic effect. Any advice please?

I have just received the Penaten cream, Do i apply it all over my face or just on oily parts of my face or on acne prone parts of my face, Thank you for your help hope this works to reduce oil and sorry do i use morning and night or just night time? Thanks again

There is no problem, both increase the concentration of coenzyme A in the skin, nevertheless you need to adjust the dose, you need them just to clear acne and anything beyond that is not required.

I mean you keep the skin neither oily nor dry, just tell me.

Hi ive been doing the Panthetine(900mg) for over a week now and my skin has cleared up 99% but strangely my skin is still real oily(even could be oilier)so its like the oil coming out is clean.. Should i add more Pathethine or just give it more time to work? Thank you

Yes, it is D-Panthenol. You can get it from Amazon, there is a cream called Penaten which is 5% dexa panthenol, this is the best concentration.

I have had taken your adivce and started taking one extra capsule of Pantethine. In total I was taking 3 x 450 split through out the day and I was also taking 3g Of Panthenic Acid.

I have seen no difference at all. If I was to lower the dose of Pantethine to 2 x 450 and up the dose of Pantenthenic Acid to 5g split in between my day. So 1 Pantenthine in morning then at lunch 2.5g of PA, then for Dinner 2.5g of PA and before I go to bed 1 capsule of Pantethine.

Will this dose be harmful to my body?? Will I need to take b complex?? or should I just give up on b5? Id say it has been a little over 2 weeks.

I have seen a slight improvement since the last time I posted on here but I am still not getting the desired results as I still have spots.

I started off with 2 grams of PA now upto 3. I was wondering can I Increase this dosage upto 4g of PA? As I feel that maybe my body needs more PA and that is why my spots are not clearing.

Panthenol is the alcohol form of PA, it is converted to PA after being absorbed from the skin. This makes you not needing to increase the dose.

I haven't seen any improvements yet. I have added b5 and it has only been 4 days but it has been 10 days since I have been on PA but my spots dont seem to be getting any less. ( i have mild acne).

Am I the unlucky one who this does not work for. I guess accutane may be my only hope but I really really do not want to take that.

It is the enzyme activity in each individual that makes us try to manipulate the B5 intake.

The recommended dose of B5 is 10 mg/day, anything beyond this dose is a studiousness and we should be careful not to flood the system, with high doses of anything however it is safe.

jhwilso1 had increased the dose of pantethine to 450x5 times and he got a clear skin, yet I do not recommend that for very long.

What is 900mg equivalent to PA? And why such a less dose? Although PA breaks down into Pantethine would it not make sence to keep the dose at the same level as it is only a less step in the pathway but not less in dose??

Calcium Pantothenate is the pharmaceutical form of Pantothenic acid. it is OK.

As for the dosages you may need to try them starting with the recommended doses. Start with 2 capsules of Pantethine and 2 grams of PA.

ATP is available inside all ages, and what I can say to you is that what you describe is promising, either you proceed as you do now for some more time, or you take PA with Pantethine.

I have been reading 'jhwilso1' posts and it seemed that PA and Cystien worked better for him. But after reading so much of the post all the questions have confused me as I am completely new to all of these vitamins.

I hope you can shed some light here.

I am currently taking 450mg of Pantethine twice a day and I am thinking of taking an extra which makes it 3 x 450.

After only 3 days my skin feels tight and dry im not too sure if this is the Pantethine working. Skin seems to look somewhat clearer but still have a few spots on my forehead and cheeks.

I have ordered vit b5 which I plan to take 1 x 500mg a day, Will I need b Complex vitamins??

You said that ATP is present in people upto the age 20 (i think) I am 24 so does this mean I need to supplement with ATP? What about Cystien? If my body has not produced enough Cystien does this also mean Pantethine will not be effective?

I was reading up on pantethine as discovered a post;

'pantothene is used for different purposes than pantothenic acid. it's main use seems to be for helping the body burn energy quicker thus lowering the level of cholesterol in the body. So it is mainly used by people with high choleterol. Pantothenic acid does nothing to the bodies cholesterol levels so cannot be used for this purpose. Also pantothene has shown no signs of lowering sebacous gland production in the body. So no pantothene is really no use for acne or oily skin problems.'

I have just purchased pantethine and after reading this and other posts regarding pantethine it has made me begin to think maybe i should use pantothenic acid instead??? Please could you elaborate on this matter for me?

I have tryed everything and very frustrated at this moment as nothing is working. Also I take actyl L carnitine on the days I go gym as it is also meant boost your energy levels and use up the stored fat (I hope this is correct). Pantothenic acid is a precursor of Pantetheine. Pantethine contains 2 molecules of this Pantetheine linked together.

You better start with Pantethine as it will save you taking Cysteine, and it has good results as well.

thank you very much for your enlightening posting. As in my country Europe Pantethine is not sold as medicine, I only use Pantothenic Acid and L-Cysteine. My skin is getting better with 1,5g PA and 1g L-Cysteine. As I use this regimen for less than a month, I wonder whether it will be getting better even without Pantethine.

The question is why do we make all these efforts to erase fatty bumps? they will show again frequently!

My point is that we keep acne at bay with the least efforts and costs, whenever it comes.

From what I understand Pantethine turns into Coenzyme A. Coenzyme A is responsible for oxidizing or burning up fatty acids which in turn reduce oil or sebum on the skin. L-Carnitine is a transporter of fatty acids. It transports fatty acids to be burned up as energy. As an analogy. Coenzyme A is the furnace that burns up fatty acids. Carnitine is the delivery truck that brings fatty acids to the furnace.

Doesn't it make sense to take both of them?

In addition to taking the Methionine. I was thinking about adding L-Carnitine. Supposedly it helps transport fatty acids across the mitochondrial membrane where they can be oxidized.
