Canceling or Closing Florida Tax Accounts with the Florida Department of Revenue

Due to the poor status of the economy, many businesses have to close. If you have found yourself in this situation, it may be unclear as to how your tax accounts should be cancelled with the Florida Department of Revenue. You cannot cancel your tax accounts with the Florida Department of Revenue over the phone. The Florida Department of Revenue requires tax account cancellations to be done in writing and have given three options as to how this can be done. Remember, cancelling your tax accounts with the Florida Department of Revenue does not automatically cancel any other accounts you may have open with other Florida agencies or departments. You will have to contact each Florida agency or department regarding account closing procedures.


Canceling Tax Accounts Using the ‘Closing or Sale of Business’ Form or ‘UCS-3’ Form

If you file paper sales and use tax returns (Dr-15 or DR-15EZ), in your tax return booklet is a ‘Closing or Sale of Business’ form. This form is located on the backside of the ‘Change of Address or Business Name’ form, which is the second form in your booklet (behind the resale certificate and before the filing checklist). This form can be used to cancel your sales and use tax account and any other tax accounts you may have. If you file unemployment taxes, there is a form UCS-3, which can be used to cancel your unemployment tax account and any other tax accounts you may have. Mail either form to:

Canceling Tax Accounts Using the Florida Department of Revenue’s Website

The fastest way to cancel a tax account is through the Florida Department of Revenue’s website. Go to www.myflorida.com/dor, under the E-services heading, click on the More E-services link. Under the General Tax Administration section, click on the link Change Address or Account Status. Scroll down past the green ‘check’ and red ‘x’ to begin completing the form. Once the form is completed, click on the Submit button at the bottom of the page. Your online request should be processed by the Florida Department of Revenue after three business days.


Mail a Cancellation Letter to the Florida Department of Revenue

The last option is to mail a letter to the Florida Department of Revenue requesting that your tax accounts be cancelled. Here’s a checklist of what items to include in your letter:

Put request on a company or personal letterhead

Provide all tax account numbers

Provide a cancellation date (can be a current, past, or future date)

Include your signature, title, and contact number

Or you can hand deliver your letter to your local Florida Department of Revenue service center.


Tips

If mailing a cancellation request, call the Florida Department of Revenue after a week of mailing date to confirm account has been cancelled. If submitting request online, call the Florida Department of Revenue after three business days of submission date to confirm account has been cancelled.

The Florida Department of Revenue does not send confirmation through mail or email that accounts have been cancelled unless you request so.

The Florida Department of Revenue does not impose a fee to cancel account.
